Opportunity:

RN - Hospice

Job Details:

* Weekday schedule Monday through Friday from 8:00 AM to 5:00 PM
* Occasional back-up for weekends and on-call responsibilities
* Manage an assigned caseload of patients as a Primary Registered Nurse
* Lead the interdisciplinary team caring for assigned patients
* Provide compassionate hospice care in a variety of settings including private homes, nursing homes, group homes, and assisted living facilities
* Help patients identify their personal goals of care and support caregivers in caring for their family members
* Apply strong clinical skills as a generalist across all diseases with a specialty in symptom management
* Demonstrate excellence in assessment and critical thinking
* Deliver personalized, patient-centered care in an environment that fosters individualized approaches
