Opportunity:

Clinical Care Leader (CCL) - Post Acute

Job Details:

* Work shifts from 8:00 AM to 4:30 PM
* On call coverage required every 5th weekend
* Post acute care setting
* Guarantee of 36 hours per week for twelve-hour shifts or 40 hours per week for eight to ten hour shifts
* Work week runs Sunday through Saturday
* Utilize the nursing process including assessment, diagnosis, outcomes/planning, implementation, and evaluation to provide individualized nursing care in the home setting
* Collaborate with inter-professional colleagues to plan, implement, and evaluate care
* Document resident history, symptoms, medications, and care provided in compliance with applicable standards and regulations
* Utilize technology to enhance quality resident care and collaborate with inter-professional team members
* Effectively delegate to and supervise unlicensed personnel
* Assume overall nursing accountability for resident assignments
* Serve in leadership roles on department and organizational task forces and committees
* Demonstrate competency and practice within the full scope of nursing expertise and knowledge
* Utilize appropriate age and population specific standards as designated in the specific care area or unit
* Charting system: Point Click Care (PCC)
* Resident Assessment Instrument (RAI) process experience preferred but not required
* BSN not required
* Required certifications: BLS Certification
