Opportunity:

RN - Case Management

Job Details:

* 26-week contract assignment
* Seasoned travelers only - first-time travelers are not accepted
* No floating requirement
* Day shift patient ratio of 1:25; weekend patient ratio of 1:25
* Weekend rotation required (every other weekend)
* Must commit to working 3 of the 5 holidays: Thanksgiving, Christmas Eve, Christmas Day, New Year's Eve, and New Year's Day
* RTO restricted to no more than 7 days per contract
* All RTO and specific scheduling requests must be submitted upfront during submittal
* Dress code: Business attire or scrubs
* Charting system: Epic
* Orientation: 40 hours
* Minimum 2 years of acute care case management and discharge planning experience required; behavioral health experience does not apply
* Must have hospital experience
* California state license required; pending license accepted but must be licensed upon start date
* Patient age groups served: Neonates, adolescents, adults, and geriatrics
* Experience required in assessing patients, care coordination, discharge planning, and utilization review
* Ability to organize and participate in family meetings and contribute to treatment plan development
* Experience with HMOs, IPAs, and similar organizations required
* Knowledge of adoption/foster care, drug/alcohol abuse treatment options, homelessness resources, mental health crisis support, advance directives, POLST, and protective services processes required
* Must be able to independently arrange DME, SNF placements, and ambulance transport
* Interdisciplinary support includes physical therapy and social services
* Computerized documentation required
* Required skills include: Acute hospital, admission criteria, benefits eligibility, care coordination, CMS knowledge, Department of Health standards, discharge planning, disease management, DRG, emergency department, ICU, MS, needs assessment/DME ordering, plan of care, prior authorizations, surgical services, and Joint Commission/Core Measure/National Safety Goals
