Opportunity:
RN - Case Manager
Job Details:
* Provide skilled nursing and case management for assigned patients in their place of residence
* Admit patients onto services and coordinate and integrate care to ensure continuity across disciplines
* Utilize the nursing process including assessment, planning, and implementation of care through an inter-disciplinary team approach
* Observe, assess, evaluate, and plan interventions related to patients' physical and psychosocial statuses, including changes in condition or reactions to treatments and medications
* Oversee care provided by Home Health Aides and CNAs and evaluate compliance with the plan of care
* Provide appropriate symptom management and promote optimal health and quality of life through teaching, counseling, and referrals
* Maintain accurate and complete patient records in accordance with agency standards and OASIS requirements
* Participate in team scheduling coordination and on-call rotation schedule as needed
* Assess and evaluate patient needs via telephone and conduct home visits as required
* Participate in quality improvement activities and attend in-service training as assigned
* Position may start as a Visiting RN and transition to Case Manager role with the same clinical responsibilities applying to both designations
* Current RN license in the State of Montana required (BSN preferred)
* Valid driver's license, appropriate auto insurance, and reliable transportation required
* 2 years of recent acute care, home care, or hospice experience preferred, including venipuncture, catheterization, wound care, and/or parenteral therapy
* HCHB experience strongly preferred
* Rapid Response Crisis position
