Opportunity:

RN - Case Manager

Job Details:

* Minimum 3 years of acute care case management experience required
* California case management experience is required due to the nature of the role
* Active California RN license required - no pending or temporary licenses accepted
* BLS certification required
* Epic charting system experience highly preferred
* No weekend shifts required
* No floating required
* No on-call required
* No scheduling requests can be accommodated - must be flexible
* Patient age groups include adolescents, adults, and geriatrics
* Patient caseload of 15 patients per day
* Skills required include discharge planning, care coordination, utilization of InterQual criteria, utilization of Milliman guidelines, ICD-10 coding, DRG knowledge, and HIPAA compliance
* Interdisciplinary support includes social services and transportation
* 2 supervisor or higher references from within the past year required at time of submission

This is a large acute care hospital facility with a daily census of 255 patients, offering a collaborative interdisciplinary environment that includes social services and transportation support. The facility serves a diverse patient population spanning adolescents, adults, and geriatrics, and operates with a strong focus on care coordination, utilization management, and regulatory compliance standards.
