Opportunity:
LVN - SNF
Job Details:
* All nursing staff are required to wear Caribbean Blue scrubs
* Work under the direction of the charge RN to provide nursing care to acute patients
* Report to the Director of Nursing (DON) and work in conjunction with physicians, nursing staff, residents, families, and other facility departments
* Conduct individualized patient assessments and prioritize data collection based on patient's immediate condition or needs
* Develop individualized plans of care reflecting collaboration with other members of the healthcare team
* Perform appropriate treatments as ordered by the physician in an accurate and timely manner
* Perform tasks such as EKGs, blood draws, and IV starts
* Adhere to end of shift walking rounds at 6:30 AM and 6:30 PM to maintain continuity of care
* Administer medications accurately and follow facility policy and procedures for medication and narcotics control
* Initiate emergency resuscitation measures according to appropriate resuscitation protocols
* Provide individualized patient and family education
* Maintain confidentiality in matters related to patients, families, and facility staff
* Assist with orientation of new personnel and staff education as needed
* Must hold a current California (CA) nursing license
* Current BLS level CPR certification required
* Strong assessment skills and ability to function independently
* Must be comfortable working with patients who may be confused or verbally and physically abusive
* Strong interpersonal, problem-solving, and written and oral communication skills required
