Opportunity:

RN Case Manager

Job Details:

* Day shift position working Monday through Friday from 7:30 AM to 4:00 PM
* No on-call, no holidays, and no weekend requirements
* Assess discharge needs and social determinants of health for acute medical and surgical patients
* Develop and coordinate discharge planning and referrals
* Manage a typical caseload of approximately 20 patients per day
* Minimum 3+ years of acute care case management experience required
* Must reside outside a 60-mile radius of the facility
