Open Position: PT - Physical Therapist

Estimated Gross Weekly Pay: $2532.40

Opportunity:

Physical Therapy - Home Health

Job Details:

* Monday through Friday, 8:00 AM to 5:00 PM with rotating weekend shifts (8:00 AM to 5:00 PM)
* 5 shifts of 8 hours per day
* 6.5 points required per day
* Rotating holiday schedule required
* Perform start of care (SOC), resumption of care (ROC), recertifications, evaluations, and daily visits
* Treat patients with diagnoses including CHF, orthopedic conditions, and pneumonia
* Cover home health care setting independently in the field
* Clinical manager or educator available 24/7 for emergencies and 8:00 AM to 5:00 PM for general support
* 2-week orientation with a work coach, including supervised visits
* Phone and tablet provided for use in the field
* Charting system: Homecare Homebase (HCHB) - experience required
* Home health experience required
* Must have active license in hand upon submission
* No new graduates preferred
* Dress code: Scrubs or business casual
* Required certifications: BLS (AHA)
