Opportunity:

RN - Long-Term Care / Skilled Nursing Facility

Job Details:

* Utilize the nursing process and critical thinking in collaboration with interdisciplinary team members in caring for patients
* Perform assessments, planning, intervention, and evaluation of care
* Administer prescribed medications and treatments in accordance with approved nursing techniques, policies, procedures, standards of care, and state laws
* Ensure all documentation is accurate and comprehensive in accordance with established standards of care
* Develop, initiate, and execute a patient plan of care according to individualized patient needs as prescribed by physician and/or hospital policy
* Provide education to patients and families at the appropriate level of understanding
* Participate in department or unit performance improvement activities, which may include shared governance
* Utilize clinical expertise to provide guidance to other units and departments in support of the patient's longitudinal plan of care
* Utilize critical thinking regarding patient safety and patient experience, manage complex situations, and delegate to other staff appropriately
* Escalate to the appropriate chain of command as needed
* Engage in personal development and support continuous learning and growth including developing or revising evidence-based practice guidelines
* Support and participate in peer review and peer feedback opportunities
* Current Illinois (IL) nursing license required
* CPR/BLS certification required
* Must be a graduate of an accredited nursing program; education verification required
