Opportunity:
RN - Case Management
Job Details:
* Seasoned travelers only - first-time travelers will not be considered
* No floating expected
* Day shift patient ratio of 1:25; weekend patient ratio of 1:25
* Minimum 2 years of acute care case management and discharge planning experience required; behavioral health experience does not apply
* Weekend rotation required to meet department needs (every other weekend)
* California state license required; pending license accepted but must be active upon start date
* RTO restrictions: no more than 7 days; must commit to working 3 of the 5 major holidays (Thanksgiving, Christmas Eve, Christmas Day, New Year's Eve, New Year's Day)
* Dress code: business attire or scrubs
* Charting system: Epic
* Orientation hours: 40 hours
* Patient age groups: neonates, adolescents, adults, and geriatrics
* Daily census: 25 patients
* Interdisciplinary support includes physical therapy and social services
* Required skills include: assessing patients, care coordination, discharge planning, and utilization review
* Must have experience with HMOs, IPAs, and similar managed care organizations
* Must be able to organize and participate in family meetings and contribute to treatment plan development
* Knowledge of adoption/foster care, drug/alcohol abuse treatment options, patients experiencing homelessness, mental health history or crisis situations, advance directives, POLST, and protective services processes required
* Must be competent with arranging DME, SNF placements, and ambulance transport independently
* Experience with CMS, Department of Health regulations, DRG, prior authorizations, and The Joint Commission core measures required
* Computerized documentation experience required
* Travelers may be expected to float between nearby sister facilities; refusal to float may result in termination of assignment and/or forfeiture of guaranteed hours

The facility utilizes Epic for charting and provides interdisciplinary support through physical therapy and social services.
