PT - Home Health

Job Details:

* Day shift position working 8:00 AM to 4:30 PM, Monday through Friday
* No weekend or holiday requirements
* Treat 5-6 units per day including start of care visits, discharges, recertifications, and regular visits
* All patients are seen in a home health setting
* Cover counties including Braxton, Gilmer, Calhoun, Nicholas, Webster, Clay, Randolph, and Pocahontas
* Ideal candidate to help cover Webster and Nicholas Counties, though scheduling will be coordinated based on therapist location to optimize mileage and productivity
* Treat common diagnoses including general orthopedic conditions (hips, knees), generalized weakness, strengthening, and cardiac rehab
* Self-scheduling is allowed with flexibility to move patients based on geographic area within initial and reassessment guidelines
* Work alongside a team of 4 PTAs
* Document using Epic charting system - orientation to Epic will take place at the Gassaway office
* Dress code: Navy scrubs or khaki bottoms with navy polo
* Required certifications: BLS Certification
* Must meet all PT licensure requirements for practicing in the state of West Virginia
* Cancellation policy: 2 shifts per 13-week contract

This home health organization serves patients across eight counties in West Virginia, providing comprehensive rehabilitative care directly in patients' homes. The department is supported by an experienced team including physical therapist supervisors and four physical therapist assistants, fostering a collaborative and supportive clinical environment. The organization utilizes the Epic documentation system and tailors orientation to each clinician's individual needs to ensure a smooth transition into the role.
