Case Management
Job Details:
* Acute care case management position with a patient caseload of up to 25 patients
* Day shift scheduling with a patient ratio of 1:20
* Every other weekend required with availability for holidays - must commit to working 3 of 5 holidays (Thanksgiving, Christmas Eve, Christmas Day, New Year's Eve, New Year's Day)
* No floating requirement during standard shifts; however, may be required to cover up to 2 units on weekends with a focus on discharge planning
* 2 years of acute care case management and discharge planning experience required
* Travel experience required; first-time travelers will be screened
* California state license required; pending license accepted but must be active upon start date
* Epic charting system experience required
* Dress code: Business attire or scrubs
* Competent with InterQual guidelines highly preferred
* Experience with admission criteria reviews including ED, Mother Baby, and NICU
* Experience with care coordination, patient assessments, and discharge planning required
* Knowledge of HMOs, IPAs, Medicare, and MediCal required
* Ability to independently arrange DME, SNF placements, and ambulance transport
* Knowledge of adoption/foster care, drug/alcohol abuse treatment options, patients experiencing homelessness, mental health, advance directives, POLST, and protective services process
* Ability to organize and participate in family meetings and contribute to treatment plan development
* Computerized documentation required; experience with Epic system required
* Interdisciplinary support available including IV teams, physical therapy, respiratory services, pharmacy, social services, and rapid response teams
* Acute behavioral health experience does not meet acute hospital requirements
* RTO restrictions: No more than 7 days; all RTO must be requested upfront during submittal
