Opportunity:

RN - Hospice

Job Details:

* Day shift position
* Manage a caseload of typically 12?18 patients, ensuring continuity of care
* Conduct initial and ongoing assessments of patients' physical, emotional, psychosocial, and spiritual needs
* Develop and update individualized plans of care in coordination with physicians and the interdisciplinary team
* Deliver skilled nursing care including medication management, pain and symptom control, and wound care
* Educate patients and families on disease process, symptom expectations, medications, and hospice philosophy
* Provide emotional support and crisis intervention to patients and families as needed
* Communicate regularly with physicians, social workers, chaplains, hospice aides, and other team members
* Participate in interdisciplinary team meetings to review and revise care plans
* Ensure timely and accurate documentation in the electronic medical record (EMR) in compliance with regulatory requirements
* Ensure compliance with all Medicare, Medicaid, and state/federal hospice regulations
* Participate in on-call rotation and respond promptly to after-hours patient needs
* Mileage reimbursement provided in addition to compensation
* Hospice experience required
* Minimum two years of recent field experience required
* Bachelor's Degree in Nursing (BSN) preferred; five years of hospice nursing experience preferred
* Valid Driver's License required; must be at least 21 years of age
* Current automobile insurance and reliable automobile required
* Required certifications: CPR or BLS (from approved vendor per policy)
* Current RN licensure in state of practice required
