Opportunity:
Case Manager
Job Details:
* Minimum 2 years of acute hospital case management experience required
* Travel experience required; first-time travelers will not be considered
* Patient ratio of 1:35, with a caseload of up to 25 patients daily
* Required to monitor up to 2 units on weekends with a focus on discharge planning
* Every other weekend required; must be available for holiday coverage including 3 of 5 holidays (Thanksgiving, Christmas Eve, Christmas Day, New Year's Eve, New Year's Day)
* RTO restricted to no more than 7 days
* Scheduling type is variable; float between sister facilities nearby may be required
* Scrubs or business casual attire required
* Charting system experience required; Epic system used
* Computerized documentation experience required
* MSW certification required
* Compact license must be in hand at time of submission
* 2 references required covering 1 year within the last 3 years
* Key responsibilities include discharge planning, care coordination, biopsychosocial assessments, and participation in treatment plan development
* Must have knowledge of drug/alcohol abuse treatment options, patients experiencing homelessness, patients with mental health history or in crisis, and POLST
* Must be able to organize and participate in family meetings
* Performing dual roles of care coordination and discharge planning
* Interdisciplinary support available including IV teams, physical therapy, respiratory services, pharmacy, radiology, and more
* Serves patient age groups including neonates, infants, adolescents, pediatrics, adults, and geriatrics
