Job Details:
* Seasoned travelers only - first-time travelers will not be considered
* Patient ratios: 1:25 on days and weekends
* Weekend rotation required (every other weekend)
* Must commit to working 3 of the 5 holidays: Thanksgiving, Christmas Eve, Christmas Day, New Year's Eve, and New Year's Day
* California state license required; pending license accepted but must be active upon start date
* Must have a minimum of 2 years of acute care case management and discharge planning experience
* Hospital experience required; behavioral health experience does not apply
* Dress code: Business attire or scrubs
* Charting system: Epic
* Orientation: 40 hours
* Patient age groups: Neonates, adolescents, adults, and geriatrics
* Perform patient assessments, care coordination, discharge planning, and utilization review
* Organize and participate in family meetings and contribute to treatment plan development
* Experience with HMOs, IPAs, and similar organizations required
* Knowledge of adoption/foster care, drug/alcohol abuse treatment options, homelessness resources, mental health crisis support, advance directives, POLST, and protective services processes required
* Must be able to independently arrange DME, SNF placements, and ambulance transport
* Interdisciplinary support includes physical therapy and social services
* Computerized documentation required
* Required skills include: acute hospital care, admission criteria, benefits eligibility, care coordination, CMS knowledge, Department of Health regulations, discharge planning, disease management, DRG knowledge, emergency department experience, ICU experience, needs assessment and DME ordering, plan of care development, prior authorizations, surgical services, and Joint Commission/Core Measure/National Safety Goals
* No floating required at contracted facility; however, travelers may be expected to float between nearby sister facilities
* Maximum of 7 days of requested time off; all RTO must be submitted upfront at time of submittal
