Opportunity:

Physical Therapist - Home Health

Job Details:

* Provide one-on-one physical therapy care for patients in their home setting
* Assess patient conditions and develop individualized plans of care to maximize mobility and independence
* Collaborate with interdisciplinary team and colleagues to ensure patient needs are met
* Perform standard therapy measurement techniques including range of motion, strength, and balance tests
* Utilize a variety of therapeutic modalities to maximize care provided to patients
* Manage patient cases including OASIS documentation
* Vermont PT licensure required
* Home health experience and OASIS experience required
* McKesson/Netsmart EMR experience strongly preferred
* No requested time off - quick start position
