Opportunity:

Home Health PT - Mt. Angel, OR and Surrounding Areas (Northern Territory; Woodburn, Mt. Angel, Aurora, Molalla, Gervais, Silverton)

Job Details:

* 8-hour day shifts
* No weekend requirements
* No on-call requirements
* Visit up to 6 patients per day with 30 productivity points standard (1:1 ratio)
* Service area includes Mt. Angel and surrounding northern territory communities including Woodburn, Aurora, Molalla, Gervais, and Silverton
* Occasional maximum radius of roughly 60 miles (60-90 minute total drive time) per day
* System groups clients to minimize drive and windshield time
* Personal transportation required
* Laptop and work cell phone provided for documentation and work communication
* Charting completed via EPIC Remote Client for Home Health (on laptop) and Rover phone app; OASIS documentation required
* Attire is business casual or scrubs (no jeans, no specific scrub color required)
* Free parking available
* Mileage reimbursement provided at IRS standard rate
* Patients include adults and geriatric populations
* Skills required include geriatrics, home health, OASIS, and orthopedics
* 1 year of skilled home health experience required
* Epic charting system experience preferred
* Required certification: BLS
* Compact license must be in hand at time of submission
* Guaranteed hours subject to facility calling off up to one shift per pay period (every two weeks)

This home health facility provides skilled in-home patient care across a network of communities in the northern Oregon territory. The organization serves adult and geriatric populations and utilizes modern technology including EPIC and OASIS-based documentation systems to support clinical staff. The facility prioritizes efficient patient scheduling to minimize travel time for providers and offers a supportive interdisciplinary environment with access to interpretation services.
