Opportunity:

Case Management

Job Details:

* Minimum 2 years of case management experience required; acute care hospital experience is a hard requirement and must be current within the last year
* Travel experience required; first-time travelers accepted as a screen with clinical approval
* Every other weekend (EOW) required
* No on-call required
* May float between like units based on department needs
* Charting system: Epic (experience required)
* Patient caseload of up to 25 patients
* Serves patient age groups including adolescents, adults, pediatrics, and geriatrics
* Patient ratio of 20 during days
* Interdisciplinary support includes Social Services
* Required skills and experience include: HMOs/IPAs, advance directives, POLST, protective services process, discharge planning, care coordination, appeals and denials, benefits eligibility, prior authorizations, DRG, CMS/Medicare/Medicaid, HIPAA guidelines, needs assessment/DME ordering, emergency department, ICU, MS, SDU/PCU/IMC/Obs, and pediatrics
* Compact license must be in hand at time of submission
* 2 professional references required covering 1 year within the last 3 years
* Must commit to working 3 of the 5 holidays: Thanksgiving, Christmas Eve, Christmas Day, New Year's Eve, and New Year's Day
* All RTO and scheduling requests must be submitted upfront at time of submittal
* Free parking provided on first day; assignment parking issued during orientation
