Opportunity:

Care Manager

Job Details:

* Variable 5-day workweek with department operations occurring 7 days per week
* Days off will vary and are not guaranteed to be back-to-back, though every effort will be made to schedule consecutive days off when possible
* Must commit to working 3 of the 5 holidays: Thanksgiving, Christmas Eve, Christmas Day, New Year's Eve, and New Year's Day
* Patient ratio of 1:25 with a daily census of up to 25 patients
* Patient population includes adults and geriatrics
* May be required to float to the Laguna facility
* Scrub color: Turquoise or business casual attire required
* Minimum 2 years of acute medical Care Manager experience performing care coordination required
* Travel experience required - no first-time travelers accepted
* California state license required; pending license accepted but must be active by start date
* Epic charting system experience required - hard stop requirement
* Trauma Level I and Level II experience preferred
* Assess patients, coordinate care, and manage discharge planning independently
* Organize and participate in family meetings and contribute to treatment plan development
* Experience with HMOs, IPAs, and related managed care organizations required
* Knowledge of adoption/foster care, drug/alcohol abuse treatment options, patients experiencing homelessness, mental health crisis, advance directives, POLST, and protective services required
* Competent in arranging DME, SNF placements, and ambulance transport independently
* Experience with appeals and denials, CMS regulations, discharge planning, disease management, DRG, HIPAA guidelines, needs assessments, and OSHA compliance required
* Computerized documentation required
* Case Managers do not perform utilization review
* RTO requests limited to no more than 7 days; all RTO must be requested upfront during submittal

Facility:

This is an acute care facility providing comprehensive medical services to adults and geriatric patients. The facility operates seven days a week and supports interdisciplinary care coordination across multiple departments. With access to specialty services and a focus on quality patient outcomes, the facility maintains high standards for care management and utilizes Epic as its primary electronic health record system.
