Open Position: RN - Case Management

Estimated Gross Weekly Pay: $2849.40

Opportunity:

RN - Case Manager

Job Details:

* Minimum 3 years of acute care case management experience required
* Active California RN license required - no pending or temporary licenses accepted
* BLS certification required
* Flu vaccination required - no exemptions
* EPIC charting system experience highly preferred
* No weekend shifts required
* No floating required
* No on-call required
* No scheduling requests can be accommodated - must be fully flexible
* Patient ratios: 15 patients per day, 40 patients on weekends
* Serves patient age groups including adolescents, adults, and geriatrics
* Skills required include discharge planning, care coordination, utilization of InterQual criteria, utilization of Milliman guidelines, plan of care, ICD-10 coding, and HIPAA guidelines
* Interdisciplinary support includes social services and transportation
* 2 supervisor-level references from within the past year required at time of submission
* Maximum pre-approved time off of 2 weeks; no time off within the first 2 weeks of assignment
