Opportunity:

RN - Case Manager

Job Details:

* Minimum 3 years of acute care case management experience required
* California RN license required and must be active at time of submission - no pending or temporary licenses accepted
* BLS certification required
* Flu vaccination required with no exemptions
* Epic charting system experience highly preferred
* California case management experience required due to the nature of the role
* No weekend shifts required
* No floating required
* No on-call required
* No scheduling requests can be accommodated - must be fully flexible
* Patient ratio of 15 patients per day
* Serves patient age groups including adolescents, adults, and geriatrics
* Experience with discharge planning, care coordination, and utilization of InterQual and Milliman guidelines preferred
* Knowledge of CMS, HIPAA, ICD-10 coding, DRG, and Joint Commission standards preferred
* 2 supervisor-level references from within the past 1 year of employment required at time of submission
