Opportunity:

RN - Case Management

Job Details:

* Day shift position working 5 x 8-hour shifts from 8:00 AM to 4:30 PM with no set days of the week
* Minimum 2 years of recent experience in case management or related field required
* Epic electronic health record experience required
* Behavioral health background preferred
* Works across two adult mental health units - one Medical Behavioral unit and one adult Psychiatric unit
* No on-call responsibilities
* Current BLS certification required
* Registered Nurse (RN) licensure in state of practice required
* Bachelor's Degree in Nursing (BSN) preferred
* Case management certification preferred
* Leads coordination of patient care with interdisciplinary team, monitoring appropriateness and timeliness of care
* Ensures interdisciplinary care plan is consistent with patient's clinical course, continuing care needs, and covered services
* Discusses with physicians the appropriateness of resource utilization, treatment plans, estimated length of stay, and discharge planning
* Focuses on complex patients including frequent emergency department utilizers, chronic pain patients, substance abuse patients, and homeless patients
* Coordinates transfers of patients to tertiary centers including transfer of necessary patient information
* Manages frequent emergency department visitors by conducting focus study reviews of previous and current admissions
* Provides oversight and collects data required for regulatory and accreditation compliance
