Job Details:

* Monday through Friday schedule from 8:00 AM to 4:30 PM
* Coordinate management of care and ensure optimum utilization of resources, service delivery, and compliance with external review agencies
* Provide ongoing support and expertise through comprehensive assessment, care planning, plan implementation, and overall evaluation of individual patient needs
* Enhance quality of patient management and satisfaction to promote continuity of care and cost effectiveness through integration of case management, utilization review and management, and discharge planning
* Serve as a patient advocate through resource utilization, discharge planning, and addressing holistic needs of the patient
* Minimum 2 years of recent case management and discharge planning experience in a hospital setting required
* California Social Work license required
* BLS certification required (must be through the American Heart Association)
