Open Position: PT - Home Health

Estimated Gross Weekly Pay: $2194.20

Opportunity:

PT, OT, Speech - Marian & Arroyo Grande (home based care)

Job Details:

* 8-hour day shifts with flexible scheduling, majority of work hours Monday through Friday 8:00 AM to 5:00 PM
* Home health care setting with assignment to a designated geographical region
* Every 3rd weekend requirement
* Minor holidays required, major holidays only if needed
* Business attire with white lab coat required, no jeans permitted
* Charting performed using Cerner EMR system
* PT/OT patients typically include orthopedic, stroke, joint, and spinal injury diagnoses
* Speech therapy patients may present with a wide variety of diagnoses
* Must have experience with OASIS (government mandated form)
* Minimum one year of experience working in PT, OT, or Speech required
* Home health setting experience preferred but not required
* Speech therapists: CCC-SLP and/or CCC-A credentials preferred
* Required certifications: BLS Certification
* Orientation period of 1.5 to 2 weeks
* Hospital may cancel up to 4 shifts per 13-week assignment, or up to 3 shifts for assignments less than 13 weeks

The facility utilizes Cerner for electronic medical record documentation and supports a collaborative care model with both contract and permanent staff including rehab aides, OTAs, and PTAs.
