Physical Therapist - Home Health

Job Details:

* Full-time position working day shifts
* Average patient visit is 45 minutes allowing for meaningful therapy sessions
* Evaluate patients' need for physical therapy and develop appropriate treatment plans
* Carry out treatment programs in accordance with agency policies and professional standards of care
* Communicate and coordinate treatment activities with interdisciplinary team members and patients
* Document evaluations, treatment goals, and objectives
* Teach patients and family members the rationale for treatments and procedures
* Screen, evaluate, re-evaluate, and treat patients across multiple service locations
* Work with broad independence while remaining part of a cohesive patient-care team
* Bachelor of Science in Physical Therapy required
* Must hold an active Physical Therapist license in Vermont and New Hampshire, or an unencumbered compact license
* Minimum of one year of experience as a Physical Therapist in a home healthcare setting required
