Opportunity:

Physical Therapist - Home Health

Job Details:

* Provide physical therapy services within the legal scope of practice following all state licensure requirements
* Conduct initial evaluations, progress notes, 30-day reassessments, and 60-day recertifications in accordance with Medicare and state standards
* Develop, implement, and update individualized plans of care aligned with physician orders and patient needs
* Deliver skilled interventions and maintenance therapy programs as permitted by state defined scope of practice
* Directly supervise Physical Therapist Assistants (PTAs) and Home Health Aides in compliance with state practice acts
* Consult with physicians and interdisciplinary team members regarding treatment adjustments and patient progress
* Educate patients and caregivers on home exercise programs, mobility, and adaptive strategies to promote independence and safety
* Prepare and submit OASIS assessments, recertifications, transfers, resumptions of care, and discharges within agency-defined timeframes
* Apply ICD-10 coding accurately for compliant documentation
* Present in-service programs periodically to agency staff and contribute to staff education initiatives
* Participate in Quality Assessment and Performance Improvement (QAPI) activities
* Document clinical interventions, care coordination, and communication in a timely manner
* Participate in interdisciplinary meetings, case reviews, and continuing education opportunities
* Work independently with minimal daily oversight within a 45-mile radius of the Albuquerque area (including Tijeras, Rio Rancho, Bernalillo, Bosque Farms, Los Lunas, and Belen)
* Maintain compliance with HIPAA and all privacy and confidentiality standards
* Proficiency with organizational technology systems including communication, documentation, productivity, and data management platforms required
* Graduate of an accredited physical therapist curriculum required
* Current Physical Therapist state licensure required
* Two years of clinical experience preferred; home health experience a plus but not required
* Current CPR certification required
* Reliable transportation, valid driver's license, and auto insurance required for home visits
* Basic computer proficiency and willingness to learn EMR systems and OASIS documentation required
