Opportunity:
RN - Case Manager
Job Details:
* Work with physicians and multidisciplinary team members to develop individualized plans of care for assigned patients
* Monitor patient progress toward desired outcomes through assessments and patient records
* Identify and resolve barriers that hinder effective patient care
* Actively participate in the discharge planning process
* Complete comprehensive assessments per regulated timelines and determine patient eligibility for case management
* Develop and implement case management plans in collaboration with patients, caregivers, physicians, and other healthcare professionals
* Perform ongoing monitoring of care plans to evaluate effectiveness, document interventions, and suggest changes accordingly
* Maintain ongoing patient caseload for regular outreach and management
* Promote integration of services including behavioral health care and long-term support services to enhance continuity of care
* Position may require working beyond a standard 40-hour workweek, including weekends and after-hours shifts
* On-call work may be required to respond promptly to organizational, patient, or employee needs
* Minimum 2 years of nursing experience required
* Utilization review experience required
* 2 years of acute care hospital experience as a case manager preferred
* Bachelor's Degree in Nursing preferred
* Required certifications: American Heart Association BLS
* Montana Registered Nurse license required
* Candidate must reside at least 90 miles away from the facility to be considered
