Opportunity:

Physical Therapist Assistant (PTA) - Home Health

Job Details:

* Flexible scheduling with full-time or part-time hours available
* Provide skilled in-home therapy services under the direction of a licensed Physical Therapist
* Carry out established treatment plans involving therapeutic exercise, gait training, balance activities, and functional mobility interventions
* Monitor and communicate patient progress and changes to supervising Physical Therapist
* Encourage patient participation and reinforce goals
* Complete timely and accurate documentation in EMR
* Collaborate with therapists, nurses, caregivers, and other care team members
* Work independently and adapt professionally in home-based settings
* Valid driver's license, reliable transportation, and auto insurance required
* Current Colorado Physical Therapist Assistant (PTA) license in good standing required
* Must be a graduate of an accredited Physical Therapist Assistant program
* Strong communication and documentation skills required
* Paid drive time and mileage reimbursement provided
