Opportunity:

RN - Case Management

Job Details:

* Monday through Friday day shifts from 8:00 AM to 4:30 PM (8-hour shifts with 30-minute lunch break)
* No on-call requirements
* Business casual dress attire required; no scrubs
* BSN required
* Active California RN license required
* AHA BLS certification required
* Minimum 2 years of clinical hospital RN experience required
* Minimum 2 years of hospital case management experience including discharge planning and utilization review required
* Community inpatient hospital experience preferred
* Travel experience required; no first-time travelers
* Knowledge of InterQual is a plus
* Epic charting system experience preferred
* Patient caseload ratio of 20 patients per day; 40 patients on weekends
* Work with adult and geriatric patient populations
* Skills required include discharge planning, medical necessity determination, DME ordering, InterQual criteria utilization, and CMS compliance

Facility:

This facility is a full-service community inpatient hospital with 225 beds and a daily census of approximately 120 patients. The organization provides comprehensive medical services to adult and geriatric populations and offers interdisciplinary support including social services. The case management department works collaboratively across departments to ensure effective discharge planning and utilization review in alignment with evidence-based guidelines.
