Opportunity:

Home Health PT - West OR and Surrounding Areas

Job Details:

* 8-hour day shifts (facility cannot accommodate 4x10s or 3x12s)
* May visit up to 6 patients per day with 30 productivity points standard
* No weekend requirements
* No on-call requirements
* 1 year of skilled home health experience required
* First-time travelers accepted
* Locals accepted within a 50-75 mile radius
* Guaranteed hours: facility may call off up to one shift per pay period (every two weeks)
* Oregon licensure required
* Traveler must have their own reliable transportation
* Occasional maximum travel radius of roughly 60 miles (60-90 minute total drive time) per day; system groups clients to minimize drive and windshield time
* OASIS experience required
* Charting system: EPIC Remote Client for Home Health (on laptop) and Rover (phone app)/OASIS
* Epic charting experience preferred
* Attire: Business casual; scrubs allowed but not required and no specific color required; no jeans
* Parking: Free
* Mileage reimbursement at IRS standard rate
* Required certifications: BLS
* Preferred certifications: Manual Therapy, Hand Therapy, Wound Care
* Patient population: Adults and Geriatrics
* Patient diagnoses include: Dementia, Parkinson's, ALS, MS, orthopedic post-op, and post-stroke
* Preferred skills: Billing, Home Health, and OASIS documentation
* Required skills: Lifting and transfer, ordering supplies, and scheduling appointments
