Home Health RN - Full Time

Job Details:

* Full-time position working Tuesday through Saturday
* Provide individualized goal-oriented nursing care using the nursing process in accordance with physician orders
* Serve as Case Manager in cases where this position is the lead discipline
* Deliver home health and hospice care to individuals of all backgrounds
* Provide health promotion, long-term care, and maternal-and-child health services
* Position is categorized as Rapid Response Crisis urgency
