Job Details:
* Home health setting serving adult and geriatric patient populations
* Patient ratio of 1:6
* Territory coverage radius of 40?80 miles
* Mileage reimbursed at $0.55 per mile
* Weekends not required
* No on-call requirement
* No float requirement
* Guaranteed 3 shifts per 13-week contract; cancelled shifts must match the regular shift length assigned
* Maximum of 7 days of requested time off per contract
* Scrub color: any color or style as long as it is professional and clean in appearance
* Charting system: Epic (experience required)
* Minimum 2 years of experience required; first-time travelers not accepted
* Travel experience required
* Illinois license in hand required at time of submittal
* Required certifications: BLS, PTA, Wound Care
* Required skills include: previous home care experience, strong critical thinking and assessment skills, communication skills, collaboration with PT, organizational skills, time management, and flexibility
* Clinical skills required: Cardiopulmonary, Geriatrics, Medical Surgical, Orthopedics, Home Health, Adaptive Devices, AROM/AAROM/PROM, Assistive Devices, Body Mechanics, Identifying Environmental Barriers, Pain Assessment, Sensory Response, Skin Integrity, Strength Testing, Vital Signs, Breathing Techniques, Gait Belt, Rolling Walker, Sliding Boards, Standard Walker, Supplemental Oxygen, Total Hip Replacement, Total Knee Replacement, Home Exercise Program, Lifting and Transfer, Medications, Order Supplies, Patient and Family Education, Scheduling Appointments, and Telephone Calls
* Interdisciplinary support available including Physical Therapy, Interpretation Services, and Social Services

Clinicians utilize Epic for charting and are expected to bring strong clinical expertise and independence to each patient visit.
