Home Health RN

Job Details:

* Provide care for patients in their home setting within Plymouth County, Iowa service area
* Scheduled day shifts, Monday through Friday
* Position offers 32-40 hours per week
* Participate in on-call rotation for the department, one in four nights, weekends, and holidays
* Minimum 2 years of experience performing OASIS assessments required
* Experience with care planning for in-home patients required
* Previous experience with Epic EMR system is a plus
