Opportunity:

Rehabilitation Services - SNF/ICF

Job Details:

* Generally scheduled five days per week with likely inclusion of one weekend day
* Possible floating to other units or facilities based on census and staffing needs
* Must be available to work Hawaii State and Federal Holidays as necessary based on facility operations
* Provide rehabilitation services in a Skilled Nursing Facility (SNF) and/or Intermediate Care Facility (ICF)
* Perform evaluations, reassessments, treatments, and other clinical services consistent with licensure, discipline, and scope of practice
* Develop, implement, and carry out individualized treatment plans in collaboration with supervising therapist, physician, and interdisciplinary team
* Implement evidence-based therapeutic interventions to maximize function, independence, communication, mobility, swallowing, and quality of life
* Educate residents, families, and caregivers regarding treatment plans and patient progress
* Complete accurate, timely, and compliant documentation including progress notes, changes in plan of care, and discharges
* Supervise therapy assistants when applicable to role
* Minimum two years of experience in SNF/ICF setting preferred
* Current Basic Life Support (BLS) certification required
* Current State of Hawaii license in good standing required prior to start of assignment
* Orientation provided for rehabilitation department, EMR documentation (Net Health Rehab Optima and Epic), and facility workflows
