Job Details:

* Day shift schedule
* Mileage reimbursement provided in addition to base rate
* Minimum 2 years of recent home health or acute care experience required
* Bachelor's Degree in Nursing (BSN) preferred
* Required certifications: BLS (from an approved vendor)
* Valid Driver's License required with minimum age of 21 years
* Current automobile insurance and reliable vehicle required
* Current RN licensure in the state of practice required
* Plan, organize, and direct home care services to meet individual patient needs
* Manage a patient caseload in an effective and efficient manner
* Complete initial and ongoing comprehensive assessments of patient needs at appropriate time points
* Implement and update plans of care in accordance with identified patient needs
* Coordinate patient care with other disciplines and integrate input from care team members
* Counsel and educate patients and families in meeting healthcare and nursing needs
* Accurately document all pertinent patient care information in accordance with established policies and procedures
* Provide direct and/or indirect skilled, safe, and quality care independently while collaborating with core disciplines
