Job Details:

* Work Monday through Friday, 7:00 AM to 4:00 PM or 8:00 AM to 5:00 PM with no weekend or on-call requirements
* Float between inpatient and outpatient care within the same facility under one hospital license
* Treat 7 treatment hours per day with a productivity expectation of 24 units in an 8-hour day
* Holiday rotation may be required, to be discussed during interview
* Serve patients in a 25-bed critical access hospital treating skilled nursing patients with an average length of stay of 14 days
* Common diagnoses include general SNF, wounds, ortho, neuro, debility, fractures, orthopedic injuries, neck pain, and a small percentage of neurological cases and amputations
* Inpatient and outpatient experience required; wound care experience preferred but not required
* Perform patient evaluations and develop individual treatment plans and goals to maximize functional outcomes
* Actively participate in discharge planning, implement treatment programs, and reassess treatment results
* Chart using MEDITECH system
* May be asked to precept as needed by the unit
* Maroon scrubs required (not provided by facility)
* Required certifications: BLS, Texas state license, and national license
* New graduates accepted with a minimum of six months of experience

This critical access hospital operates a 25-bed inpatient unit focused on skilled nursing care, alongside outpatient therapy services housed within the same facility. The rehabilitation team includes full-time staff physical therapists who provide support and handle specialized cases, with complex cases referred to a sister facility nearby. The facility fosters a collaborative environment with supportive nursing and therapy staff and utilizes an interdisciplinary team approach to patient care.
