Opportunity:
Physical Therapist
Job Details:
* Previous skilled nursing experience required
* Perform evaluative and therapeutic procedures under physician or accepted healthcare provider orders
* Evaluate residents and establish therapeutic plans of care including functional long- and short-term goals, frequency and duration of services, and therapeutic modalities
* Monitor and evaluate treatment effects and progress treatment programs toward established functional goals
* Document all evaluative and therapeutic interactions in the Electronic Medical Record per department procedures
* Recommend and obtain appropriate assistive devices and provide instruction in their use
* Provide education to residents and/or caregivers and document results of teaching
* Collaborate with Physical Therapist Assistants and other therapy professionals to ensure quality resident care
* Understand and comply with Medicare, Medicaid, and other reimbursement regulations
* Supervise assistant-level staff activities in accordance with state mandates and professional organization standards
* Provide mentorship to new staff during orientation
* Maintain department cleanliness and follow all infection control procedures
* Bachelor's degree required; Master's or Doctorate preferred
* Active Physical Therapist license in applicable state required
* Basic Life Support (BLS) certification preferred
* Prior experience in long-term care or skilled nursing facility preferred
* Must be able to lift up to 50 pounds; above 50 pounds requires a 2-person lift
* Must be able to perform tasks involving a combination of sitting, standing, walking, bending, stooping, twisting, and reaching
