RN - Home Health

Job Details:

* Monday through Friday, 8:00 AM to 5:00 PM with weekend rotation required
* 5-day, 8-hour shifts per week
* Holiday rotation required
* No on-call requirements
* Case manage roughly 30 adult patients per day
* 6.5 points per day required including start of care, recertifications, and daily visits
* Must have Home Care Home Base (HCHB) experience and home health experience - no exceptions
* Must have 1 year of experience on a Med/Surg floor in a hospital setting
* Must hold active state nursing license in hand upon submission
* Required certifications: BLS (AHA)
* Scrubs required with no color requirement
* Phone and tablet will be provided
* 2-week orientation with a work coach including visits with a preceptor
* Clinical manager or educator available 24/7 for emergencies or 8:00 AM to 5:00 PM for general support
* Charting system: Homecare Homebase (HCHB)
