Opportunity:

PTA - Home Health

Job Details:

* Monday through Friday, 8:00 AM to 5:00 PM with weekend rotation required
* 5 eight-hour days per week
* Holiday rotation required
* Must have home health experience and Home Care Home Base experience to be considered
* Must have license in hand upon submission
* Treat patients with diagnoses including CHF, orthopedic conditions, and pneumonia
* Perform start of care (SOC), resumption of care (ROC), recertifications, evaluations, and daily visits
* Target of 6.5 points per day
* Work independently in the field with access to a clinical manager or educator available 24/7 for emergencies or 8:00 AM to 5:00 PM for standard support
* 2-week orientation with a work coach including supervised visits
* Charting system: Homecare Homebase (HCHB)
* Phone and tablet will be provided
* Dress code: Scrubs or business casual
* Required certification: BLS (AHA)
