Estimated Gross Weekly Pay: $1974.80

Opportunity:

Home Health RN

Job Details:

* Monday through Friday, 8:00 AM to 5:00 PM with weekend rotation required
* 5 shifts of 8 hours per day
* Holiday rotation required
* No on-call responsibilities
* Case manage approximately 30 adult patients per day using a 6.5 point daily system
* Perform start of care visits, recertifications, and daily visits
* Cover Marion, Washington, Taylor, LaRue, Hardin, Meade, and Breckenridge Counties
* Home Health only does not cover Hospice
* Resource support available from clinical manager or educator 24/7 for emergencies or 8:00 AM to 5:00 PM for standard needs
* 2-week orientation with a work coach and supervised visits provided
* Phone and tablet will be provided
* Scrubs required with no color requirement
* Must have Home Care Home Base (HCHB) charting experience no exceptions
* Must have a background in home health no exceptions
* Must have a minimum of 1 year of experience on a Med/Surg floor in a hospital setting
* Must be licensed in the state of work
* Required certification: BLS

This home health organization provides skilled nursing care directly to patients in their homes across a multi-county coverage area in Kentucky. The facility focuses exclusively on home health services for adult patients, offering comprehensive case management and clinical support. Traveling nurses in the field have access to a clinical manager or educator around the clock, ensuring continuous support and a strong framework for delivering quality patient care.
