Opportunity:
PT - Home Health
Job Details:
* Work Monday through Friday from 8:30 AM to 5:00 PM
* See 3 to 5 patients per day depending on visit type
* Up to 2 Start of Care visits per day, or 3 to 4 evaluations, or any combination totaling 3 to 5 visits
* Start of Care visits are weighted at 3.75 to 4 hours total including travel, documentation, and case management
* Provide physical therapy treatments in patients' homes, Assisted Living facilities, Senior Apartments, and Board and Care homes
* Treat a variety of diagnoses including Ortho, Cardio, Pulmonary, Oncology, Neuro, Geriatric, and Amputations no pediatrics or Hospice
* Supervise up to 2 PTA/COTAs
* Must be able to work in home environments that may be cramped, hot, cold, or cluttered
* 1 weekend shift per month requested but not required
* Holiday work is required with flexibility available if time off is requested in advance
* 3-week orientation process includes online learning and field precepting; solo work expected by end of week 2 or week 3 at latest
* Charting system: Homecare Homebase (HCHB); prior experience preferred
* Home Health experience including OASIS required
* 2 years of experience in a healthcare setting required; community or home health experience preferred
* Valid current driver's license and proof of automobile insurance required
* Good oral and written communication skills and strong organizational skills required
* Required certifications: BLS (AHA) and valid NPI number
* California PT license required
* Dress code is business professional; scrubs recommended but not provided must wear closed toe shoes, no jeans, no tank tops, no shirts with logos
* Mileage will not be reimbursed
Facility:
This home health organization operates multiple service teams covering an expansive service territory, primarily supporting patients from area hospitals and community physicians. The facility manages patient care across several regions, with territories grouped strategically to minimize travel time and promote care continuity. The therapy program is overseen by an RN Manager with additional support from a Regional Rehab Leader and Educator for therapy-related guidance.
