Job Details:
* Work day shifts from 8:30 AM to 5:00 PM with typically one Saturday per month rotation
* Minimum of 2 years of experience as a Physical Therapist in an acute, home care, or rehab setting required
* Required certification: BLS
* Service area covers Yolo County including Woodland, Davis, Yolo, Dunnigan, Arbuckle, West Sacramento, Natomas, Knights Landing, Capay, Vacaville, Winters, and Esparto
* Dress code: Scrubs are not required; if scrubs are not worn, slacks (khakis acceptable) with a polo or button-up shirt are required; any color scrubs are acceptable if chosen
* Charting system: Homecare Homebase (HCHB)
* Provide physical therapy services to patients and clients according to written physician plans of care
* Assess, evaluate, and update therapeutic, rehabilitative, and functional status while participating in the development of total care plans
* Evaluate home environments and assess muscle strength, mobility, gait, and range of motion with attention to rehabilitation potential
* Instruct patients, families, and caregivers in the use and care of therapeutic appliances
* Report patient reactions to treatment and changes in condition to the physician
* Initiate physical therapy programs and instruct other personnel and family members in relevant phases of physical therapy
* May train patients in the use of prosthetic devices
* Supervise homemakers and Home Health Aides participating in patient rehabilitation regimens
* Identify patient, family, and caregiver needs for additional home health services and refer as necessary
* Prepare and submit clinical and progress summaries based on attainment of goals
* Provide physical therapy consultation to home families and caregivers when indicated
* Participate in peer consultation process and ongoing staff competency assessments
* Comply with Title 22 Medicare Conditions of Participation, Medi-Cal, OSHA, and other applicable regulations
* Must have good oral, written, and organizational communication skills
* 2 supervisor references from submitting modality required at submittal (1 within 12 months and 1 within 3 years)
