RN - Home Health

Job Details:

* Monday through Friday schedule from 8:00 AM to 5:00 PM
* Home health care setting serving four counties (Noble, Lagrange, Steuben, and DeKalb), primarily rural areas with mid-sized towns
* Facility uses a point system equivalent to 4-6 patient visits daily
* Tablet provided to track time and visits
* Business casual or hunter green scrubs with white or black top required
* Common cases include elderly falls and fractures
* Must complete Oasis Assessments and Start of Care cases
* Must have home health experience as well as Home Care Home Base experience
* Charting system: Homecare Homebase (HCHB)
* Required certifications: BLS (AHA)
* Valid nursing license must be in hand upon submission
