Open Position: PT - Physical Therapist

Estimated Gross Weekly Pay: $2549.60

Opportunity:

Physical Therapist - Home Health

Job Details:

* Monday through Friday, 8:00 AM to 5:00 PM with rotating weekend shifts (8:00 AM to 5:00 PM)
* 5 eight-hour days per week
* Must complete 6.5 points per day including Start of Care (SOC), Resumption of Care (ROC), recertifications, evaluations, and daily visits
* Holiday rotation required
* 2-week orientation with a work coach, including a few supervised visits
* Home Health care setting covering patients with diagnoses including CHF, orthopedic conditions, and pneumonia
* Charting system: Homecare Homebase (HCHB) prior experience with this system is required
* Home Health experience required
* No new graduates preferred
* Clinical manager or educator available 24/7 for emergencies and 8:00 AM to 5:00 PM for general support
* Phone and tablet will be provided
* Either scrubs or business casual attire is acceptable
* Required certifications: BLS
* Active license in hand required upon submission
* BSN not required
