Open Position: PT - Physical Therapist

Estimated Gross Weekly Pay: $2441.60

PT - Home Health

Job Details:

* Work 8-hour day shifts from 8:00 AM to 5:00 PM
* 5 days per week schedule with rotating weekends and holidays
* Provide home health physical therapy services across designated zip code coverage areas
* Treat an average of 6.5 patients per day
* Perform Start of Care (SOC), Resumption of Care (ROC), Recertifications, Evaluations, and standard visits
* Common diagnoses include CHF, orthopedic conditions, and pneumonia
* Minimum 1 year of experience required; new graduates not preferred
* Clinical manager or educator available 24/7 for support
* 2-week orientation with preceptor provided
* Dress code: Scrubs or business casual
* Phone and tablet provided
* Required certification: BLS
