LTC LVN/LPN

Job Details:

* 4x12 or 3x12 variable shift schedule (traveler's choice)
* Shifts will be either days or nights based on facility needs at time of start date - caregivers are not typically asked to rotate between day and night shifts
* Assignment duration is traveler's choice of 13 or 26 weeks
* Guaranteed 60 hours per 2-week period; traveler may be called off for 1 twelve-hour shift per 2-week period
* Patient ratio of 1:20 on days, nights, and weekends
* Minimum 1 year of experience required; previous travel experience required
* Long-term care experience required; LTAC experience preferred
* No on-call required
* Clinician will float within scope to meet facility needs
* Locals not accepted - candidate's permanent address must be at least 50 miles from facility
* No more than 1 week of RTO for a 13-week contract; no more than 2 weeks of RTO for a 26-week contract
* Must be willing to work at least 2 of the 3 major winter holidays (Thanksgiving, Christmas, New Year's); all RTO must be submitted at time of submittal
* Scrub color: Any
* Charting system: ECS; charting system experience required
* Free parking on site
* Required certifications: BLS; Alaska license must be in hand at time of submission
* All caregivers must obtain Alaska Food Worker Certification during orientation on-site; there is a one-time $10 fee for this certification
* Required skills include: admissions, ambulation, blood glucose monitoring, feeding tube care and management, Foley catheter insertion (male and female), IM & SQ injections, isolation precautions/PPE, mouth care, NG tube/G tube, oxygen administration, vital sign monitoring, wound care, phlebotomy lab draws, respiratory treatments (nebulizers), peritoneal dialysis (subcutaneous only), and Accuchecks
* Special equipment includes: lifts, vital sign machines, glucose monitors, Sara Steady, pacemaker transmitter, and EKGs
* Patient age groups: adults and geriatrics
* Patient diagnoses include: dementia, Alzheimer's, TBI, plegics, cognitive impairments, and physical impairments
* Facility will arrange a vehicle for traveler use during assignment; traveler must provide a valid driver's license and proof of insurance (including rental car coverage) prior to operating the vehicle
* Vehicle is available for use no earlier than 72 hours before contract begins and no later than 24 hours after contract ends
* Vehicle is for local use only and must not be taken off-road or outside a 30-mile radius from facility
* Vehicle must be returned clean and with a full tank of gas

Facility:

This is a long-term care facility with a total of 40 staffed beds and 10 rooms. The facility provides care for adult and geriatric patients with a variety of diagnoses including dementia, Alzheimer's, traumatic brain injuries, and other conditions appropriate for long-term care. Interdisciplinary support includes social services, and the facility maintains a daily census of 40 patients.
