Opportunity:

Hospice Nurse

Job Details:

* Admit patients to hospice service and conduct comprehensive assessments of physical, psychosocial, environmental, safety, and developmental needs
* Develop individualized plans of care based on patient and family assessments
* Document observations and interventions related to the patient's environment
* Coordinate care with a multidisciplinary team
* Participate in the hospice program's quality assessment and performance improvement program
* Conduct follow-up visits as needed
* Cover territory including Millbrae, Burlingame, San Mateo, Foster City, Belmont, Redwood City, and neighboring cities as needed
* Minimum one year of hospice experience required
