Opportunity:

RN - Hospice

Job Details:

* Admit patients to hospice service and conduct comprehensive assessments of physical, psychosocial, environmental, safety, and developmental needs
* Develop individualized plans of care based on patient and family assessments
* Document observations and interventions related to the patient's environment
* Coordinate care with a multidisciplinary team
* Participate in the hospice program's quality assessment and performance improvement program
* Conduct follow-up visits as needed
* Serve a territory covering San Francisco, Brisbane, Daly City, Colma, South SF, Pacifica, and neighboring cities as needed
* Minimum of one year of hospice experience required
