Opportunity:

REGISTERED NURSE (RN) - Home Health

Job Details:

* Full-time position with a Monday through Friday day schedule plus on-call rotation participation
* In-person home health position conducting independent visits in patient homes
* Perform comprehensive patient assessments and develop individualized plans of care
* Provide skilled nursing care and appropriate interventions
* Monitor patient progress and report changes to the care team
* Educate patients and families on medications, treatments, and care plans
* Coordinate care with LPNs, therapists, caregivers, and providers
* Complete timely and accurate documentation in EMR systems (HCHB preferred)
* Active Registered Nurse (RN) license in the State of Colorado or active NLC compact license required
* Valid driver's license, reliable transportation, and auto insurance required
* Strong assessment and critical thinking skills required
* Ability to work independently and manage a structured caseload
* Comfort with documentation and EMR systems required
* Home health experience preferred but not required
* Mileage reimbursement provided
