Job Details:

* OASIS experience required
* Hospice coverage while on call required
* Utilize the nursing process to assess patient and caregiver needs to provide quality care in accordance with evidence-based practice, state and federal regulations
* Develop and communicate realistic plans of care for each patient while ensuring patients and caregivers are included in the care plan process
* Serve as Case Manager for assigned patients and actively manage caseloads and visit times in accordance with agency productivity standards
* Complete OASIS documentation in accordance with agency policies, procedures, and state and federal regulations while maintaining up-to-date knowledge of changes to OASIS documentation requirements
* Implement medical plans of care, nursing interventions, and procedures to ensure safe and effective quality of care
* Maintain up-to-date high-tech skills including IV use and maintenance, wound VAC, and complex wound care while ensuring infection control standards are maintained
* Collaborate with LPNs to ensure plans of care are followed, delegate tasks, review documentation, and annotate visit notes in accordance with department policies and procedures
* Evaluate implemented plans of care including identified problems, care provided, and patient responses to ensure overall quality of care
* Maintain accurate and up-to-date documentation of nursing care provided for effective integration of information across the healthcare team
* Follow agency protocol with McKesson processes including locking down visits, follow-up notes, and visit completion
* Perform patient and caregiver education on topics including medication side effects, treatment options, discharge instructions, and reportable conditions
* Identify and document interdisciplinary needs of patients including physical therapy and social work, and communicate information to other disciplines in a timely manner
* Accurately complete a variety of agency, state, and federal forms including HHABN and Medicare Notice of Non-Coverage in a timely manner
* Carry out visits in a time-effective manner and ensure supervisory visits are not combined with routine visits when necessary
* Assess patients' needs for HCA services, develop and coordinate plans of care with patients and caregivers, and provide ongoing supervision
* Transmit patient incident reports to QI Specialist in a timely manner as necessary
* Coordinate safe patient discharges and transfers and ensure all appropriate paperwork and patient information is processed in accordance with department policies and state and federal regulations
* Coordinate patient care with physicians, providers, team members, and vendors in support of the recommended plan of care
* Participate in monthly peer review audits and provide and accept constructive feedback in a professional manner
* Proactively schedule, coordinate, and facilitate case conferences with manager and team members to maximize effective communication
* Advocate for patient care issues to ensure overall quality and type of care is sensitive to each specific patient and family's needs
